This will allow us to create your file and provide our dental professionals with a detailed overview of your general and oral health, as well as your medical history. This questionnaire, which must be updated every 5 years or according to the dentist's judgment, covers various aspects of your health, past dental treatments, medications you are taking, etc. We can send it electronically before your first appointment so you can bring it with you when you come. Before your consultation, your dentist will carefully review your information to better assist you. Your answers will remain confidential and can only be accessed by authorized personnel.
